Audit Report

The Audit Report is a key feature of Junipa, providing a comprehensive view of all recorded evidence for NCCD compliance. It is designed to support schools during the NCCD audit process by presenting evidence in a structured, audit-ready format.

Use it when you need the broad school view: which students have evidence, which records look thin, which adjustment levels are represented, and which rows need follow-up before census week.

Accessing the Audit Report

  1. Navigate to Reports > Audit Report from the main menu.
  2. The report displays all students with recorded evidence, organised by adjustment level.

What the Audit Report Shows

Each entry in the audit report includes:

What to check first

Start with the records that would be hardest to explain to another person. These are often the students with support activity but unclear evidence.

Look for:

  • Students with an adjustment level but very little evidence.
  • Students with evidence that does not describe the adjustment.
  • Students with old evidence and no recent classroom examples.
  • Students marked not included even though support activity exists.
  • Records that rely on vague language such as "helped in class".
  • Adjustment levels that have changed during the year.
  • Evidence that has no attachment when a file would make the record clearer.

Filtering the Report

You can filter the audit report to focus on specific data:

  • By Student -- Search for a specific student to view all their evidence.
  • By Adjustment Level -- Filter to show only students at a particular NCCD level (QDTP, Supplementary, Substantial, Extensive).
  • By Date Range -- Narrow the report to a specific time period (e.g., Term 1, or the full calendar year).
  • By Staff Member -- View evidence recorded by a specific teacher or case manager.

Teacher-friendly review questions

When you share an Audit Report gap with a teacher, keep the request specific. Useful questions are:

  • What adjustment is happening for this student?
  • How often does it happen?
  • What has changed for the student because of the adjustment?
  • What note, file, plan detail or work sample can show that?
  • Is the student correctly included or correctly not included?

This keeps the report from feeling like a compliance spreadsheet. It becomes a focused student support conversation.

Viewing as an Auditor

The "View as Auditor" mode presents the report in the format an external auditor would see:

  1. In the Audit Report, click the View as Auditor toggle.
  2. The report reformats to show a summary view with:
    • Student name and adjustment level
    • Total evidence count
    • Date range of evidence
    • Breakdown by adjustment type
  3. Click on any student to expand and view their full evidence log.

This view is useful for pre-audit preparation -- it allows you to see your data the way an auditor will.

What a strong record looks like

A strong student record usually has:

  • A current adjustment level.
  • A clear disability category.
  • Evidence from more than one point in the year.
  • At least some evidence from classroom teachers.
  • Attachments where the file helps explain the support.
  • Notes that describe the adjustment, not just the meeting or task.
  • A reporting decision that is deliberate, not accidental.

Evidence Log

The Evidence Log provides a chronological list of all evidence entries across the school:

  1. Navigate to Reports > Evidence Log.
  2. The log displays every evidence entry, sorted by date (most recent first).
  3. Each entry shows:
    • Date
    • Student name
    • Adjustment type
    • Brief description
    • Who recorded it
  4. Click on any entry to view the full details, including attachments.

Excluding Evidence from Reports

If an evidence entry was recorded in error or is not appropriate for NCCD reporting, it can be excluded:

  1. Open the evidence entry (either from the student's profile or from the Evidence Log).
  2. Click the Exclude from Report option.
  3. Confirm the exclusion.

Excluded evidence:

  • Will not appear in NCCD audit reports.
  • Will still be visible in the Evidence Log with an "Excluded" label.
  • Can be re-included at any time by opening the entry and clicking Include in Report.

Excluding evidence does not delete it. The entry and its audit trail are preserved, ensuring a complete record of all actions taken.

Exporting the Report

To export the audit report:

  1. Apply any filters you need.
  2. Click the Export button.
  3. Select the export format (PDF or CSV).
  4. The report will download to your device.

Exported reports are useful for sharing with auditors, school leadership, or for your own records.
