Use this when a student needs a clear plan that teachers, families and support staff can read and use.

Do this
- Open the student record.
- Start a new plan or open the current draft.
- Add the goals, adjustments and support details that staff need to follow.
- Use existing evidence to keep the plan connected to what is already known.
- Save the plan and check the next review point.
Before you move on
- The plan uses language a teacher can act on.
- Adjustments are specific enough to use in class.
- The plan has an owner or review rhythm.
